MATCH準備-面試經驗2/行程準備

有了第一次的教訓, 我把面試問題準備的更周嚴, 也是先想好很多問題來問下一個program, 我準備的習慣是把所有的問題列出來, 然後把自己的答案在電腦裡打出來, 這樣在複習自己的問題的時候才知道自己的thinking process, 因為人腦的記憶容量還是有限

在等待下一個面試的過程我去了一趟Houston 去拜訪很久沒碰面的姨媽, 看到了從來就沒見過小表妹, 在Houston 停留3天之後就飛到Chicago.先來認識一下環境.

來介紹一下行程規劃...先來介紹交通的部分...

ICIS International Student Card

到美國機票大家都知道怎麼定吧! 如果是在學學生或是快要畢業可以在畢業前申請ICIS國際學生證, 使用期限1年, 可以選到便宜又可以用很久的機票

我定機票都是定southwest airline  , 好處是如果是先2個星期前訂票的話有early bird price, 價格很便宜, 如果臨時要更改班機不用手續費, 機票錢全部轉到下一次使用, 行程可以很flexible, 我就用這種方式改過一次機票. 美國國內航空公司規定都不太一樣, 以southwest airline 在機場內有機器可以print電子機票和幫你check in luggage, 但是boarding pass 要到Gate的櫃檯跟他換才拿的到, 如果自己 run 過一次就應該知道怎麼做.....



Greyhound是最大的bus company



如果自己面試通知有很多都是在同一個地理區域, 時間又拉的很長需要來來回回, 可以考慮使用greyhound discovery pass,  , 一個月pass $439, 兩個月pass$539, 不建議買7或15天的pass因為不會比機票便宜, 坐greyhound的好處是定點多, 可以到有些飛機到不了的地方, 但是缺點是美國區域很大, 點和點之間都要坐很久, 像從Cleveland 到 Chicago就像從基隆做到墾丁一樣久, 不過像我這種省錢一族還是很划算的啦....

Megabus超好認

另外如果旅行地點是在Chicago或是NY附近, 可以參考另外一個便宜的選擇Megabus,它的招牌就是這位笑臉大叔 ,有時候價錢會低到讓你難以置信....例如Pittsburgh->Washington DC 只要$1 , Megabus減少營運成本的方式是不設公車站, 它只有公車站牌, 公車站牌的位置 可能要花時間google找一下,或是看一下 路線圖, 因為Megabus 主要是為NY或Chicago而設的, 所有的公車都會回到NY或 Chicago, 它是一定要電腦訂票, 不過這樣卻篩選掉很多游民因為他們不會用電腦訂票, 所以會使用Megabus的都是學生為主, 我自己沒有搭過Megabus, 不過聽我朋友說比greyhound舒適又有wifi .....我以後應該會嘗試一下



一般來說在美國東部交通都很發達, 但是在西部除了SF 外其他地方很多都是公車到不了的地方, 這個時候就要租車了...要先到台灣監理所申請國際駕照, 在機場附近都有租車站, 行車的方式和台灣一樣都是靠右行駛, 但是有一些規則不太一樣, 像是看到stop sign要完全停止後才能前進, 可以紅燈右轉但是要先停4秒而且要先禮讓直行車, 左轉要先轉到左轉車道等等......美國的車道跟台灣不太一樣容易搞混所以可以先找一些旅遊書入門....

另外也可以使用美國國鐵Amtrek, 在各地區也有自己的鐵路系統, 像LA的metrolink, Chicago的 Metra, NJ 的NJ transit syetem 其實都很方便, 但是路線要自己先做功課才不會拖著大行李迷路

可以去背包客棧比價

如果真的要找更便宜的Hostel....到Hostelbooker.......這個網站好處是可以付多一點錢, 取消的時候deposit可以完全退回(48小時前), 我定7天的房間取消才花我$3 ,
我第一次入境和第二次.入境都是用Tourist visa在沒工作的狀態, 要證明我是"遊客", 我曾經先定旅館準備給海關人員看我有地方住, 入境之後在把機票改期, 旅館取消, 然後去住親戚朋友家, 不過兩次入境都是超級順利拿到6個月的I94,........不要說我教壞你 ^^;   如果對visa/入境有疑問我都是去PTT的 visa版上爬文

可以上priceline.com 看有沒有意外的驚喜 , 不錯的 中文訂房網頁整理 
省錢一族又想交朋友可以去當別人家的couch potato  -couchsurfing.org , 不過自身安全要小心, 永遠都要有plan B
訂旅館有錢人有有錢人的方式, 像我這種窮人又想呆很久每天都在想撇步來省錢 lol

網路的部分
在台灣我已經習慣用小筆電配 3G 無線上網, 到美國的時候我一值在想有沒有辦法申請當地的3G無線上網又不用綁約的...結果終於被我找到了哈哈哈   Cricket Boardband    

要先買它的3G modem 然後後的月繳約$40, 可以隨時停用, 等到下次來美國在復用... 缺點是要在某些城市申辦並activate modem, 像Pittsburgh, Chicago, Houston 都有很多Cricket的店, 但是NY, LA 等就沒有, 可以先輸入要去地點的郵遞區號選擇最近的店, 一但activate之後全美都可以使用....有小筆電+3G 就天下無敵了

另外旅館/STARBUCKS都有無線上網, 不然就要到Library上網, 訊號都很強. 自己可以在Hotel 偷偷用找不用密碼的wifi, 一般都找的到

衣著的部分
11月之後美國東北部, 或中西部都很冷, 就想像如果去合歡山會穿什麼衣服去美國就帶什麼衣服吧

Chicago -美國第3大城, 但沒有LA和NY的髒亂

話說到了Chicago之後發現這個城市真的很乾淨, 每個建築物都很有特色, 交通有CTA和Metra很方便, 在路上走的人看起來都是上班族和遊客, 感覺他們都很"健康", 沒有像NY和LA 路上很多遊民和怪人....我在到一個城市之前會先看這個城市到底安不安全, (US Census Bureau  ,Most Dangerous city ), 我沒有種族歧視的意思....但是一般黑人比例正比於危險度, 我以前真的不知道Detroit, St Louis, Atlanta是危險的地方, Detroit 黑人比率是80%!.... 還有錢財不要全放在同一個地方, 皮包不要放太多錢, 可以兌換一些旅遊支票, 畢竟出門在外還是小心一點才好

MATCH準備-面試經驗

我總共接到10個面試通知, 我10/13就先飛到LA拜訪朋友順便先適應一下環境, 第1個interview本來是11/10號在chicago的St.Joseph hospital, 我也訂好了到chicago的機票, 但是就在我到美國沒多久就收到St Peter's hospital的interview invatation, 信裡直接問我要不要在10/28號面試, 沒有其他的日期可以選擇....我當時馬上說好, 但是機票旅館交通等規劃全部都被10/28的面試打亂掉了, 所以我10月底都在匆忙的面試準備過程中度過

第一站-St.Peter's hospital-10/28



St. Peter's hospital 地點在New Jersey中部, 離NYC 大概有 1個小時的車程, 因為早上7:15分就要到醫院集合, 所以我再前一天就在離醫院很近的一家旅館住一晚, 在前一天就把西裝, 領帶等都整理好第二天早上就可以直接套上.....




面試服裝到底要正式到什麼程度呢? 大概穿到成這樣應該就沒問題了, 男生的話最好是深色西裝配深色西裝褲和深色襪子,裡面在配淺色襯衫加條紋或單色領帶


第二天我 7:00到醫院大廳的時候已經看到有其他的的IMG 西裝筆挺的在大廳坐好了, 跟大廳的Program Coordinator報到之後他給我一個有St.Peter's 的資料並把我安排坐在一個印度IMG旁邊, 稍微喵一下好像很多都是IMG, 讓我稍微放心一點,



我的資料夾是用上面這種, 在台灣就買的到, 請仔細找找. 在美國可以去office depot 選購, 價格都很便宜
另外我也有看到img帶小的公事包和女生的小皮包, 看起來都不錯, 重點是正式又不笨重,
千萬不要背背包過去想說可以裝有的沒的

 
但是我一打開我的資料夾馬上傻眼, 我什麼都帶了竟然忘記把我金光閃閃的筆放進去, 這時候真的慘了, 環顧四週好像大廳裡好像沒有書店或便利商店, program coordinator 又站在那邊, 臨時跑掉又很奇怪, 我這時候偷偷的問旁邊一個素不相識的印度IMG女生有沒有多的筆, 我面試完之後會還他...., 不過還好她有多帶一支筆所以解決了我的糗態.....真是糗大了,這真的是個不太好的開始阿!!! 

等到人都到齊之後, PC 把我們帶到他們morning meeting 的地方, 大部分的resident 都已經坐定位了, 我們要自己選一個resident 坐在他旁邊一起看他們的MKSAP內專複習用書, resident 大部分都是印度面孔參著幾個AMG, 我眼尖剛好看到一個東方面孔就趕快坐在她旁邊, 一問之下原來是一個PGY1的上海姑娘, 長的很可愛看起來也很面善


St Peter's Hospital 雖然說是university hospital,但是沒有fellowship, 大部分resident都是 IMG


這時候Program director 出現了, 是個印度人看起來像個Bull Dog 一付不好惹的樣子, 他翻開了MKSAP然後開始一題一題討論內專問題, Program director 講話有印度口音說話又很快, 除了一些關鍵字其他我都聽不懂他在說什麼, 他講到一半興致一來就一個一個點名起來回答問題, 被點起來的resident 口音也是很重也不曉得在回答什麼....我在上海姑娘旁邊頭低低的很怕被點到丟人現眼, 之後有幾個Resident起來報Case也是被電的很慘.......還好轟炸對象都是目前的resident所以我是暫時safe了...., 我偷偷問旁邊的上海姑娘她怎麼聽的懂PD在說什麼? 她跟我說聽久就習慣了....唉..以後我不但要熟悉美國口音還要熟悉印度口音.....

接下來我們10個面試的人就被帶到一個討論室, PD 跑出來跟我們介紹program的特色, 介紹完之後要我們在眾人面前做個簡短的自我介紹, 還好只要介紹自己的名字還有在哪裡畢業的, 但是說實在的大家說話都很快, 感覺我的英文是所有人裡面最爛的......這個時候我的腎上腺素又開始飆起來了, 還好沒有丟台灣的臉......感覺大家再這個時候都很緊張, 只要是沒有被問問題的時候就靜啞啞的一片不敢說話, PC說我們這一組特別的安靜, 一問之下原來大家都是第一次面試啊....

後來我們就被分組帶到樓上一個一個面試, 剩下來還沒面試的在樓下跟目前的chief resident聊天, chief resident 也是印度來的, 她詳細回答我們提出的有的沒有的問題態度也很不錯, .......但是我們剩下來的一批人發現問題很快就問完了, 我也不知道可以問什麼, 有時候僵在那裡還滿尷尬的, 下次真的要多準備一些問題來問PD 或是resident..

在等待的過程中我還蠻緊繃的, 但是叫到我名字的時候我感覺整個人就輕鬆起來了, 可能是知道緊張也沒什麼用吧... 所以在PC 帶我上去的時候都在跟她聊天聊台灣的事....

第一個面試對象就是印度來的 Bull Dog Program director , 先寒喧幾句之後就開始正式面試, 顯然他對台灣的醫學教育還蠻有興趣的, 途中一直問我台灣的醫學教育如何如何.....我也儘可能的跟他描述......他接下來問我在醫院服替代役的時候碰到哪類的病人比較特別的, 我跟他說在台灣南部有一些TB 病人, 他突然問我TB 要怎麼治療? 雖然說這應該是簡單的問題但是我從來不記得那些藥要怎麼念,只好吞吞吐吐的說前兩個月用Isoniazid, Rifanpim, PZA, EMB, 才說到這裡PD 就插話跳到下一個問題了.......15分鐘的面試真的問一下就結束了,我覺個我第一次的表現還好

第二個面試對象是美國的associate program director 他對我們台灣的OCSE很有興趣, 問很多跟 step 2 CS和 OCSE的問題, 我就跟他說台灣的OSCE跟step 2 CS很像, 所以在醫學院的時候已經準備好了, 所以CS準備起來特別輕鬆 (其實一點也不輕鬆XD)..之後他問我生涯規劃5年後要做什麼等等的一般問題, 因為感覺好像是在跟他聊天 ,我覺得我第二次表現還算不錯

等到所有的面試都結束已經中午12點了, 我們吃完午餐之後就開始醫院參觀之旅, 上上下下參觀ICU, 病房, 圖書館等等............run 到這裡每個人都累壞了, 所以大概呼嚨一下就過去了, 和其他的面試者互換email之後就匆匆忙忙的離開 St Peter's hospital

MATCH準備-統計

雖然說對大部分的人來說, 算自己match上的機率多少是還太早, 但是有些關鍵時刻(決定要報名多少PROGRAM , 要不要接受PREMATCH 的時候) 還是要知道一下自己的機會才不會做出笨笨的決定


現在回答幾個常見問題? (有些是我在umsleforum 的PO文所以直接複製貼上)


1.IMG找到工作的機會有多少?
According to the NBME data non-US IMG
Match rate is 40%
But obvious they didnt calculate the IMG who get prematched, because prematched IMG will either not join in NRMP or withdraw themselves
I quote the post for ECFMG "IMG performance in 2010 Match" http://www.ecfmg.org/announce.htm#match2010


Although the majority of PGY-1 positions in the United States are filled through the Match, a significant number of IMG applicants obtain positions outside of the Match. For example, while 4,698 IMGs obtained PGY-1 positions through the 2008 Match, 7,276 IMGs entered PGY-1 for the 2008-2009 academic year.


For some programs who really like IMGs, prematch is their main way of getting IMG resident, so actually there's large propotion of IMG who get prematch, according to 2008: 34.7% , in another word, 1 out of 3 IMG who get into PGY1 from Prematch,


So the real percentage of IMG who get the PGY1 job should be higher,
My personal guess would be 60% (for 2008)


However,  I think everyone should get a little tenses because this year is more competitive than the previous few years. Acoording to 2010 match : it's 164 applicants per position for IM

But this year the rejection I get from the community programs always tells me it's >200 applicants per position. The residency slots are increase but can't keep up with the growth of IMGs.

PGY1訓練人數和報名人數, 最近幾年有競爭越來越激烈的趨勢

For all of us it's still a game of 1 or 0, you either get the job or not. So try to be as competitive as possible

Step 1分數和match到的科的關係, 線條中間的橫槓是平均分數

2.我有X個inetrview, 我找到工作的機會多高?
I like math games so I can try to calculate how many jobs I can get using expected value(期望值), it's good to know my chance!! I type out my thinking process so if you are interested you can also try to calculate too.

For IM the position: interview:application received = 1:16:164
For FM the position: interview:application received is 1:11:148
或是換成...
IM Application sent: interview=1:0.1
FM Application sent: interview=1:0.07


So I sent 88 IM and 30 FM,
on average, I would expect 8.8 IM interview (88x0.1)
and I would expect 2.1 FM interview (30x0.07)


In the matter of fact, my credential is average so I get 9 IM interview and 1 FM interview, that's pretty close.


And I can take one step further by calculate the expected value of a job after I get a known number of interview.(9 IM interview and 1 FM interview)


For IM the position: interview= 1:16
For FM the position: intervieew= 1:11
Or
IM interview: position=1:0.0625
FM interview: position=1:0.09


So I would expect 0.56 IM job and 0.09 FM job
I can only expect 0.65 job all together! It's far than enough to get 1 job.


For IMG the average value should be 0.6 job, and the expected value is not chance, so if you can get a number >1 means you are more than enough to get 1 job. However, Most of the people are < 1 so that's why I suggest take any prematch when it's offer, because the chance of not match is high.


However there's big variations of performace during the interview, so everyone could be the magic maker (match with 1 interview)

IM Interview數量 和MATCH 比率, 由圖可知IMG(Independent applicants),  有5個IV match機率50%, 有10個IV 80%, 有20個IV 接近100%, 如果考慮prematch 機率會比這些數字更高

IM Step 1分數和match比率的關係

IMG STEP2 成績, 有好成績不代表一定會MATCH但是成績不好機會會變的很低

大部分申請 IM的IMG都沒有research 或是publication

大部分的IMG都有1個以上的臨床work experience (有領薪水的就算, 所以INTERN/醫療替代役/醫院打工都算)

3. 我要報名幾個program 才夠?

依照上面的比例, 如果自己知道CREDENTIAL 不好的話, 至少要>100個PROGRAM, 尤其是今年這麼競爭的情況真的要多報一點

但是報名多少個program最大的限制因素還是錢....
最多可以報名3個speciality, 每一個speciality 分開計算

第1-10個program: $7.5, 第10-20個program:$ 8, 第 20-30個program :$15, >30個program:$25

我報了88個IM 和 30個 FM
IM的部分 :75+80+150+1375=1680
FM的部分 :75+80+150=305
總共六萬多台幣

自己看看自己的經濟情況, 咬著牙花下去吧!

MATCH準備-Program Communication Trick

記得我9月送出我的application之後我一直在等interview , 第1個interview 和第2個interview 之間相隔快1個月, 那個時候真的等的心煩氣燥, 覺得是不是少送什麼文件還是自己不夠好

那個時候我有做一些 "小動作" 來增加自己的Interview 機會, 後來證明真的很管用, 如果覺得自己 interview不夠多可以嘗試看看

1. Email 或是 Call Program: 加分指數: **

-你可以寫信給已經apply 的program敘述你對他們的program 很有興趣, 或是在十月之後寫信給沒有回應的program詢問自己的目前的Status 到底是什麼, 或是寫信告訴program 你有update哪些考試成績或是做了哪些新的observership,
如果program還沒有開始看你的 application 可能會先看你的, 不過因為有太多人在做同樣的動作了, 所以可能不會加到分, 不過至少不會扣分, 可以放膽嘗試
-我不敢打電話給program, 他們可能會覺得我很煩, 但是我有聽說有些老印打電話試圖自己敲定跟PD 的interview, 他們的下場如何我不太清楚 XD

2. Update 自己的 personal statement;加分指數; *****

ERAS的personal statement 可以在寄出第一份之後, 可以在ERAS 重新製造一份新的PS寄出去, 我在第二份新的PS裡面另外提到了我對本program的興趣 , 我記得在改過PS 之後, 在兩天內馬上收到兩個Interview invatation, 效果很強大

3. 寄卡片 加分指數:未知

Thanksgiving/Christmas 快到了可以去大賣場買量販的卡片, 或是自己design 一張送給所有沒有回應或是要interview的program, 他們會覺得你揪甘心, 不過要附上aamc number不然他們也不知道你是誰, 如果你人在台灣就寄email 賀卡吧


嗯....可能很多人看到這裡都覺得我很呆, 為什麼要做這些無謂的事情呢? 但是如果你站在program的角度, 如果有一堆人都是waitlist, 你會先選對program有興趣的人來interview還是要找一個沒有聽過的人呢? 反正寄email或寄卡片也不會花到什麼大錢, 也不會扣到分, 就加減試看看吧....總之要想一些跟別人不一樣的點子加強program對你的正面印象

另外這是我寫給我朋友的interview tips , 我把他轉貼來這邊提供給現在在準備interview 的人做參考, 我email 寫很快所以不太注意文法, 如果有英文錯誤請多包函
Here is my input for the interview. I think my trick sounds very basic but it works out with me big time.

Tip1. Be interviewer's friend-
In the 2 interviews I attend, actually they will start out with some very casual conversation, it's not really a question but try to talk with him like a friend and try to elongate the conversation so it will sound like a real chatting. So one of the interviewer starts with "today is pretty warm", and then I try to catch the chance and say that "according to my friend this year is unusually warm for Chicago , now is about 70 degree F but usually this time of the year it will go down to 40 ,etc etc". And then we talked about Taiwan, and we have some small conversation about the Taiwan/China politics, and he shares with his experice that he went to Beijin/HK and I told him my experience to China very quickly. The other interviewer is chatting with me about accupuncture ,the language which speak in Taiwan, weather in Taiwan, etc.
So those are very casual conversation and there's no way to prepare for them. Just don't be nervous at all and relax and enjoy the process. In both of my interviews, there are almost no pause and I made the interview very fluent.  You will know you are successful when interviewer start to talked a lot about themselves.
Have the eye contact, use hand gesture, smile or even laugh at appropiate situation, when they saw your positive energy and sencerity, they will love you and believe that you are the person they want to work with.

Tip 2: Remember your questions well-
You dont have to memorize every detail, just remember the concepts you want to answer so you can quickly answer the questions when asked. The more you are familair with your answer the more casual it will present. The day before every interview, try to look at those questions and think in your mind quickly what's the answer you will give. It helps me a lot.

Tip 3: Build up Story/Example database.
Take a lot of time to think a story, but work out big time, For example , when I say I am responsible, it's not persuasive enough, then you can add with your personal story about me being responsible. When you are saying those cases , it will either make the answer much more interesting, also give a vivid image to interviewer's mind. A lot of my story is fake, but if you practice enough it will definetly sound very real.
When I am practicing interview questions ,my friend he think it always sounds like a great answer if there's a story in it.  

Tip4. Sell yourself in time.
Like the tip 1, you have to be their friend, but when they are asking you some questions that is totally fun, for example, your habit, dont dwell into it and saying too much and lose the chance of other more important part, be wise, knowing they will also score you on academic part. The timing for you to sell yourself is always very limited and you have to catch it in time. In the interview, my interviewer ask you if there's anything I would like to add, I said myself being loyal, responsible and hard working, etc etc. Also I sell myself with clinical experience which they didnt mention it, they are super busy even they wont notice my clinical expericnce, so I need to kind of remind them.

Tip5. Praise a lot about the program.
That's one of the most powerful tool I have. MOST IMG forget to do this!!! You have to priase the program sencerely and let them to believe that this is the only place you would like to be. And this topic is very easy to cut into a conversation and also can be used as a good ending. You can collect some advantage of the program on the internet and prepare it well but dont let it sounds like you are reading a textbook. When I went to St joseph hospital morning meeting, they are discussing a case with parapneeumonia effusion. So the way I will priase the programs in the interview will be "I enjoy the morning meeting today and I found that the PD is encouraging resident to comment on the case and the whole process is very friendly" , "I like the way they tells me the case step by step and actually I was also shocked when I saw big loculation of pleural effusion", or you can praise about the curriculum, the teaching facility, etc. There's always endless thing you can praise them and make them think that they are really great. If you want you can even add "This program is really a great place to work and if I have the possibility to work here, I promise to be a hard working residents ...etc",

I think that's all the tip I can offer. Try it with people and see how it goes.

從9月開始....interview 就開始慢慢的飄下來...接下來就是匆忙的準備 interview trip

前天結束Saint Joseph hospital 的interview 其實有點沮喪
因為感覺我的英文是10個人裡面最爛的, 其他人不是從小在Chicago長大不然就有不錯的USCE
我的interview 應該算順利, 但是我真的覺得自己沒什麼機會了...
也沒有動力繼續其他的interview

昨天接到program director 打來的電話告訴我 prematch 了.....
真的像是從天上掉下來的禮物, 很不可思議的感覺...我只能一直不斷的說 thank you
到今天 sign 完contract 才真正放心...

我interview 之旅提前結束了
